Maybe I found the right place for some help. A couple months ago.... I bought on ebay what would appeared to be a NEW av123 'plate amp'. However it did not have a power supply. So for 25 bucks I thought I would take the gamble. It looks good but I have been trying to figure out how to power it. So I bought an antek an-5445 toroidal transformer. ( it outputs 45v).
On this AV123 plate amp one of those PCB boards says MFW-15 v2.1 (where power is terminated) and the other 2 are listed as ULW-10.
On the power board there appears to be 2 places where I need to hook power 4pins are labels HiVav- VAC VAC Hi VAC and 3 pins listed as -15v GND 15V.
I have the connectors as well.. but I think I have no idea to hook up the 4 high power leads... also I think for the 15v side I may need another PS because there are 2 more voltage coming off this AN-5445 18v pair and 12v pair but there is only 2 wires for each I would need 3 (something with a GND?)
I took some pictures because those probably explain better....
My goal is to use this and build a MFW-15 clone. I purchased a MFW-15 driver.

I'm a bit confused on what you are looking for, but it would appear you picked up some hacked sample combining the input section of a ULW-10 and the amp section shipped in the V3 MFW-15 amp. My guess is that it was Schifty trying to turn parts he still had into cash. Your best bet is to talk to murrel who should have similar amps on hand to compare with.
